Patient Care Organizer

The Patient Care Organizer can help you understand and keep track of your care at Norris Cotton Cancer Center.

When you complete one of the treatment pages, you can download a PDF file of that page to your computer and print more. You can also find copies of these pages in the Patient Library next to the Hematology/Oncology department.

To download and open a file, click on the file name below, select File > Save As, and then save it to your computer. These files are provided in PDF format (Adobe Reader required).
