We established the Lung Health and Pulmonary Nodule Clinic to serve our rural New England population. We provide potentially life-saving screening and health programs.
Our clinic can help:
- Complete the shared decision-making conversation.
- Refer patients to the Tobacco Treatment Program for counseling with certified treatment specialists.
- Review any unexpected or incidental chest or lung findings on any imaging.
- Review low-dose computed tomography (LDCT) scan results and discuss the next steps.
- Schedule further referrals or testing.
Referral information for healthcare providers
If you are a Dartmouth Health provider and would like to refer a patient for an LDCT scan or to the Lung Health and Pulmonary Nodule Clinic, submit the appropriate referral information in the electronic health record. You may contact 603-650-8537 for any assistance.
If you are a health provider outside of Dartmouth Health:
- Fill out the Radiology referral form to refer a patient for low-dose computed tomography (LDCT) screening only with the Radiology Department.
- Fill out the Lung Health and Pulmonary Nodule Clinic referral form to refer a patient for:
- Shared decision-making and tobacco cessation conversations with the patient and LDCT.
- Review of results of LDCT scan.
- The Tobacco Treatment Program.
- Management of incidental/unexpected chest imaging findings or further referrals or testing.
