The Lung Health & Pulmonary Nodule Clinic provides potentially life-saving next steps by connecting patients with the resources they need, reviewing tests and scans, scheduling further referrals and more.

We established the Lung Health and Pulmonary Nodule Clinic to serve our rural New England population. We provide potentially life-saving screening and health programs.

Our clinic can help:

  • Complete the shared decision-making conversation and tobacco cessation counseling for lung cancer screening.
  • Refer patients to the Tobacco Treatment Program for counseling with certified treatment specialists.
  • Review any unexpected or incidental chest or lung findings on any imaging.
  • Review low-dose computed tomography (LDCT) scan results and discuss the next steps.
  • Schedule further referrals or testing.

Referral information for healthcare providers

Included below is a brief guide to making referrals to our clinic for each listed service. The referral process you use depends on whether you are:

  • A Dartmouth Health provider. Use the steps in the second column.
  • A provider from an institution outside of Dartmouth Health. Use the steps in the third column.

Low-dose computed tomography scan only

To refer a patient for If you are a Dartmouth Health provider If you are a health provider outside of
Dartmouth Health

Low-dose computed tomography (LDCT) screening only with Radiology Department.

This referral assumes you have already conducted shared decision-making with the patient.

Select CT Chest Screening Lung Cancer in the electronic health record (EHR).

Fill out the Radiology referral form.

Note that you will need to attest to specific aspects of the patient medical record in the checklist on this form, including having conducted shared decision-making.

Lung Health and Pulmonary Nodule Clinic referrals

To refer a patient for If you are a Dartmouth Health provider If you are a health provider outside of
Dartmouth Health

Shared decision-making and tobacco cessation conversations with patient and LDCT. Use this referral if you would like our staff to have the conversation with your patient before scheduling screening.

Select Shared Decision Making and Smoking Cessation for Lung Cancer Screening in the electronic health record (EHR)

This selection includes ordering the LDCT.

Fill out the Lung Health and Pulmonary Nodule Clinic referral form and request Shared Decision Making and Smoking Cessation for Lung Cancer Screening as the reason for referral. This request includes ordering the LDCT.

Review of results of LDCT scan: For assistance after receiving Lung-RADS 3-4x scan results.

Select Abnormal Lung Cancer Screening Low Dose Chest CT Results in the EHR.

Fill out the Lung Health and Pulmonary Nodule Clinic referral form and request Abnormal Lung Cancer Screening Low Dose Chest CT Results as the reason for referral.

Tobacco Treatment Program

This referral can be requested independently or in conjunction with LDCT screening.

Select General Smoking Cessation in the EHR.

Fill out the Lung Health and Pulmonary Nodule Clinic referral form and request General Smoking Cessation as the reason for referral.

Management of incidental/unexpected chest imaging findings or further referrals or testing

In the EHR, select Abnormal (or) Incidental Chest or Lung Imaging or select Other and specify or call the number below.

Fill out the Lung Health and Pulmonary Nodule Clinic referral form and request Abnormal (or) Incidental Chest or Lung Imaging Findings as the reason for referral or call the number below.
